Coordination of Care - (school counselors, primary care doctors, hospital, etc.):
Phone Calls - First 15 minutes free, then $50 per 15-minute block thereafter.
Completion of Forms
Form(s) requests need to come with an email explaining what the need is and all necessary contact information for the receiver. A release of information form will be required for each new recipient to be compliant with HIPAA laws. There is a 72-hour (not including weekends) turnaround for basic 1 or 2-page document requests. Longer forms may take up to 7 business days (not including weekends) to complete.
Fee for Forms/Letters
$25 per page.
Cancellations
I understand that unexpected situations can arise. If you need to cancel or reschedule your appointment, please let me know at least 24 hours in advance. There is no charge for appointments canceled with 24 hours’ notice. Cancellations made with less notice, or missed sessions, will be charged the full fee of $175.
Medical Record Fees
No charge for medical records “that will be used for the purpose of filing a claim regarding or appealing a denial of Social Security Disability Income or Social Security Benefits under Title II or Title XVI of the Social Security Act” when requested by the patient, the patient’s personal representative, or “an employee or other representative of a nonprofit legal services entity or other volunteer or nonprofit program representing the patient.” Code of Maryland, Health–General § 4-304(c)(5).
For all other requests, paper copies will cost 76¢ per page plus the actual cost for postage and handling. Additionally, a medical provider may charge a $22.88 retrieval and preparation fee (unless the records are provided directly to the patient per federal HIPAA regulations).
For electronic copies, 57¢ per page (75% of the per page charge for paper copies), not to exceed $80, plus the actual cost for postage and handling.
